Repeat prescriptions

To request repeat prescriptions for your animal please complete the form below:

• Fields marked with an asterisk are required fields.
• We recommend at least 2 working days notice before collection so that your prescription can be checked by a veterinary surgeon.
• If you require treatments for more than one pet, please submit a separate request for each animal.
• Animals on long term medication require follow up examinations every 3-6 months depending on the severity of their condition to check for possible side effects and changes in their condition.
